Standard EFT Member Change Form
The standard member change form is for EFT Members that would like to request one of the following actions: Freeze, Unfreeze, Terminate, Update Credit Card.
BRG Waiver
The BRG waiver is required for all adults and youth participants that wish to use BRG’s facilities. For minors under the age of 18 the waiver must be completed by a parent or legal guardian. Get a head start on your first visit by filling out your waiver today!

La exención de BRG se requiere para todos los participantes adultos y jóvenes que deseen utilizar las instalaciones de BRG. Para menores de 18 años, la renuncia debe ser completada por un padre o tutor legal. ¡Obtenga una ventaja en su primera visita completando su exención hoy!
